3rd Round of Entire-Site Testing Results

September 25, 2020

Our 3rd round of entire-site testing was performed on 9/22/20.  We had no new cases of COVID-19 with either residents or staff.  We will be testing again on 9/29/20.  We are required to do entire house testing until we have gone 2 consecutive weeks with no additional cases.

9/22/2020 Results

We continue to follow the recommended guidance from the Centers for Disease Control & Prevention (CDC) and Centers for Medicare & Medicaid Services (CMS), and the Minnesota Department of Health (MDH), and take all appropriate measures possible to protect the health and wellness of all who live and work here. We test symptomatic residents and staff as symptoms arise and perform entire-site testing during an outbreak (1 or more positive cases of either a resident or staff in our care center). Should a confirmed case come from any of these tests, we would promptly notify our residents, their families and staff.

On September 17, 2020, the Center for Medicare and Medicaid (CMS) released Memorandum QSO-20-39-NH Nursing Home Visitation – COVID 19 which provides updated visitation guidelines for skilled nursing facilities; https://www.cms.gov/files/document/qso-20-39-nh.pdf. We are currently waiting for the MN Department of Health direction on this Memo before we begin working on a policy & implementation plan.

2nd Round of Entire-Site Testing Results

September 17, 2020

Our 2nd round of entire-site testing was performed on 9/15/2020. We have had 1 additional staff member and 1 contracted staff member test positive for COVID-19. We will be testing again on 9/22/2020.

We continue to follow all recommendations from the Centers for Disease Control and Prevention (CDC) and the Minnesota Department of Health (MDH). Residents are being monitored throughout all shifts. All staff continue to be screened prior to entering the building and are required to wear surgical masks and eye protection.

Due to the positive cases in our care center, we will continue suspending patio visits at this time. We continue to encourage closed window visits and virtual visits. Please contact Kim Lautizi, Activity Director klautizi@hmhc.sfhs.org, or by phone at 218-274-7715 to schedule your visit.

Testing Results: No New Cases!

September 10, 2020

Chisolm Health Center began proactively testing all residents and staff of on 9/8/2020. The results from this initial round of entire-site testing are shown below. We have had no additional staff or resident’s tests positive for COVID-19.  We will be testing again on 9/15/2020. Please note, we are required to do entire-house testing until we have gone 2 consecutive weeks with no additional cases.

We continue to offer outdoor visits, window visits and virtual visits. As a reminder, all visitors must comply with our screening protocols and other infection control measures when they visit their loved one.
